Return electrodes or grounding pads are utilized for a safe return of electrosurgical energy from the patient body back to the electrosurgical unit. They offer a large area of conductive material to return the flow of energy back to the ESU.
Electrosurgical return pads come in a variety of types ranging from disposable, single-use return electrodes which are manufactured with an adhesive conductive gel for secure patient application to split monitored pads. Also available are reusable electrosurgical grounding pads or plates used mostly in Animal Health procedures and some low power systems. The issue with a solid pad or plate is that movement off the plate or anything that produces resistance affects the flow of energy and can cause an interruption in the flow or in some cases tissue damage. There is also the risk of cross contamination so disinfecting becomes an important step in their usage. Modern single use disposable return pads are designed to be excellent conductors and they eliminate the risk of cross contamination.
Modern ESU units constantly monitor the split pad to sense the connection and will vary the flow of energy or under some conditions alarm telling the physician there is a problem. This system insures the doctor of good working equipment and allows him to focus on the outcome of his procedure or surgery.

Bipolar electrosurgery procedures do not require a return or grounding pad because the instrument used contains both poles so the current simply moves between the two ends. This works best in small areas and does shield the surrounding tissue from invovlement in the procedure.
